The case study is about a 76 year old lady who is diabetic and suffering from dementia. She also has Kidney related issues with long term catheter. She has been admitted because of dehydration, low blood sugar and high heart rate. She is suffering from malnutrition and self neglect.
She was prescribed IV fluids upon admission.
This is followed by introducing your patient including the multimorbidity/co-morbidities he/she lives with, ensuring you follow the NMC guidelines to protect the patient’s confidentiality.

Was there an MDT? Was this appropriate were the right people involved?

How did they communicate well? Were communication barriers accounted for?
Leadership and role modelling, who led the MDT meeting, and discharge meeting?

Partnership working- did the members of the team have the opportunity to present their own views?
Were the wishes of the patient considered?
Consider the value of a family-centred approach, and informed consent and decisions. Explain the role of Health education for effective recovery as well.
Was the patient safely transferred or prepared for transfer??

LO 4. Evaluate and enhance awareness of self and identify the impact of role modelling in therapeutic relationships.
Demonstrate a clear understanding of the value of the therapeutic relationship with a patient. What types of support are being offered to the patient?
Discuss advocacy, documentation, communication (with the patient and family) and mutual engagement/ NMC code.
Role modelling to be explored, the benefits of engagement with patient and family, being non-judgemental, barriers with relationship building to be identified and mutual trust to be developed.
