The case study is about a 76 year old lady who is diabetic  and suffering from dementia. She also has Kidney related issues with long term catheter. She has been admitted because of dehydration, low blood sugar and high heart rate. She is suffering from malnutrition and self neglect.

Consider the value of a family-centred approach, and informed consent and decisions.  Explain the role of Health education for effective recovery as well.

LO 4. Evaluate and enhance awareness of self and identify the impact of role modelling in therapeutic relationships.

Demonstrate a clear understanding of the value of the therapeutic relationship with a patient. What types of support are being offered to the patient?

Discuss advocacy, documentation, communication (with the patient and family) and mutual engagement/ NMC code.
